IMPORTANT:  The individual or entity submitting the application (“Applicant”) affirms that they are the proper recipient of any incentives paid by the BayREN Refrigerant Replacement Program (the Program). All incentives disbursed will be payable solely to the Applicant.

The Applicant acknowledges that:

·       By accepting benefits from this utility ratepayer-funded energy efficiency program, the project site identified in Section 2 of this application may not participate in any other utility ratepayer-funded energy efficiency program for refrigeration measures for five (5) years from the date of this application.

·       Any misrepresentation in this application may result in partial or full project rejection and may subject the Applicant to additional review or exclusion from current or future incentive programs.

The Applicant or their agent consents to providing photos and/or video needed for the Program to verify refrigerant leak repairs, other repairs, and energy-efficiency upgrades. These images, which may include equipment and interior or exterior business areas, will be used by the Program to confirm project completion and process rebates.

By signing this form, the Applicant certifies that the information and documentation provided are true, complete, and correct.

Before submitting your application, Applicant must upload all three (3) required documentation:

·       The BayREN Refrigerant Replacement (BRRR) Site Access Agreement

·       The Contractor Proposal (must include the scope of work, material and labor costs, and correct sales tax rate)

·       Photographs (must clearly show existing condition of refrigeration systems and properly labeled)
